As a registered NDIS provider, New Horizons specialize in supporting people with disability, mental health concerns, those who are aged, people at risk of homelessness, humanitarian entrants, youth, and Indigenous Australians. Based in Ryde, the role is Community Support Worker – Youth position on a Part-time basis for Max-term ending 31st December 2024 with a potential for extension with rotating shifts providing the highest quality in high needs / complex care disability services with backgrounds specifically with working with young children. The hours of work vary from weekday shifts, and weekend shift. As a community support Worker - Youth, you are required to provide support to children with an understanding of child protection services including behavioral issues, and resilience. Ideally you will have previous extensive experience working with youth and supporting them, and advocacy for children and young people. You are skilled at motivating & mentoring and can create solutions when challenges arise. Key responsibilities Collaborating with customers to develop individualized support plans that incorporate their identified wellbeing needs and goals- Regular reviews of progress.Complete comprehensive risk management plans for customerProviding support to customers to improve their independence by assisting with living skills, transport, social activities, medication and personal care, as well as other support as requiredSupporting customers to explore their practical and complex needsKeeping customer records up to date on a daily basis using online systemsDeveloping and maintaining professional relationships and networks with customers, families, caregivers and key stakeholdersSeek out innovative ways to enhance the customer journey You are Knowledge of family restoration and child protection in Australia Ability to engage with children and experience in managing challenging behaviors You are experienced in the disability sector with an understanding of recovery recovery-orientated approach Excellent documentation and negotiating skills Willingness to attend assigned training specific to the customer What you need to have Appropriate certificate level qualification in Disability or Community Services, OR equivalent relevant experience in the human services industry Valid NSW Drivers C class licence, with ability to drive vehicles in licence class, including large cars and vans.
