Community Support Worker - Chatswood.

We have an exciting permanent Full-time Community Support Worker with our HASI plus team based in Chatswood.

This position involves shift work, between Monday to Sunday, 24/7 rotating roster with active awake shifts.

Key Responsibilities  

  • Collaborating with customers to develop individualised support plans that incorporate their identified wellbeing needs and goals, regular reviews of progress.
  • Collaboration with Community Mental health staff around clinical goals set for customer
  • Complete comprehensive risk management plans for customer
  • Providing support to customers to improve their independence by assisting with living skills, transport, social activities, medication and personal care, as well as other support as required
  • Supporting customers to explore their practical and complex needs
  • Keeping customer records up to date on a daily basis using online systems
  • Developing and maintaining professional relationships and networks with customers, families, caregivers and key stakeholders
  • Seek out innovative ways to enhance the customer journey

Skill and Experience 

  • Knowledge of mental illness; preferable with experience working with people with complex mental illness
  • You are experienced in the disability sector with an understanding of recovery recovery-orientated approach

You'll also need 

  • Technical literacy with proven ability to use Microsoft Office and systems for reporting.
  • Minimum Cert IV in Mental Health or Disability or equivalent industry experience
  • Availability to work across a rotating roster
  • Valid NSW Drivers C class licence, with theability to drive vehicles in licence class, including large cars and vans.
  • Willingness to obtain an Australian Federal Police Check, WWCC, NDIS Check and first aid certificate that meets New Horizons employment criteria prior to commencing with us 
  • First-Aid Certificate
