 We are excited to offer multiple positions for both permanent and casual positions for Community Support Worker throughout our supported independent living and inclusion services.
These positions offer rotating roster options, allowing for a flexible work schedule that can accommodate various personal commitments and lifestyles. Working days – Monday to Sunday on a rotating roster.

You will provide exceptional person-centered support to our customers and empower them to achieve their identified goals and improve their well-being.

The position involves shift work, and you will need to be available to work flexible shifts that may include a combination of mornings, evenings, and weekends.

Key responsibilities

Collaborating with customers to develop individualised support plans that incorporate their identified well-being needs and goals
Providing support to customers to improve their independence by assisting with living skills, transport, social activities, medication and personal care, as well as other support as required
Provide psychosocial support to customers to explore their practical and complex needs
Keeping customer records up to date daily using online systems
Developing and maintain professional relationships and networks with customers, families, caregivers and key stakeholders
Seek out innovative ways to enhance the customer journey

 You'll also need

Technical literacy with proven ability to use Microsoft Office and systems for reporting
Minimum Cert IV in Mental Health or Disability or equivalent industry experience
Availability to work across a rotating roster
Willingness to use personal mobile phone for work purposes
Valid NSW Drivers C class license, with ability to drive vehicles in license class, including large cars and vans.
Willingness to obtain an Australian Federal Police Check, WWCC and NDIS WC to meet New Horizons employment criteria prior to commencing with us
