New Horizons exists to enhance the wellbeing of our customers and community. We are looking for a Support Worker in Bathurst, who can treat our customers as individuals and think outside the box to help them in their unique circumstances.
These roles will sit within our aged care program with rotating shifts. The hours of work vary from Monday to Sunday on rotating rosters.  
As a registered NDIS provider, New Horizons specialise in supporting people with disability, mental health concerns, those who are aged, people at risk of homelessness, humanitarian entrants, youth, and Indigenous Australians.

Key Responsibilities: -
Meet the daily care needs of residents within the Aged Care facility
Keeping customer records up to date using online systems
Providing support to customers to improve their independence by assisting with living skills, transport, social activities, medication and personal care, manual handling as well as other supports required
Provide support to residents presenting with complex care needs
Develop and maintain professional relationships with families, caregivers and key stakeholders
Complete risk assessments, support plans and other required documents in a timely manner

Skills & Experience looking for: -
Tertiary qualification in disability services, community services or a similar experience in the aged care sector
Demonstrated experience supporting people with a disability or people with complex histories / diagnosis 
Ability to make decisions and know when to escalate to safeguard customers wellbeing 
Have a valid NSW Drivers C class licence and ability to drive a van
